United States - Greater Minneapolis-St. Paul Area - Gather Health, a nanotechnology-based primary healthcare provider that champions socially supported and holistic care, has raised $15 million in a Series A funding round led by Khosla Ventures, Maverick Ventures, and Commonwealth Care Alliance. The funding will support the expansion of Gather Health's innovative primary care model across the United States.
Gather Health was founded by Brent Asplin, a physician and healthcare leader with decades of experience in the delivery of exceptional care. The company's primary care model leverages a wrap-around care team approach that transcends the walls of doctors' offices to provide in-clinic, at home, or virtual care depending on the unique medical and social needs of each patient.
Each dedicated care team consists of a Primary Care Provider, a Guide Partner to provide social support and help navigate the healthcare system, and a Medical Technician Partner to provide at-home visits. Gather Health also offers transportation to patients for medical visits, a neighborhood office with space for classes and social events, and technology-enabled care designed to help patients in the comfort of their homes.
Gather Health opened its first primary care practice in Quincy, MA in early 2023 and plans to launch other locations and markets in 2024.
"We are thrilled to have the support of Khosla Ventures, Maverick Ventures, and Commonwealth Care Alliance as we work to transform the way primary care is delivered in this country," said Brent Asplin, co-founder of Gather Health. "Our innovative model empowers patients to take control of their health and provides them with the support they need to thrive."
